Upon your first visit to our office for chiropractic or massage, you will be asked to complete some paperwork. These forms include a patient history form, privacy policy form (our complete privacy practices documentation is available by request), and a payment plan form.

For chiropractic patients there is also a terms of acceptance form to sign that details the services provided by your chiropractor. From this page you may download these forms and complete them prior to your initial visit to help save some time. You are also welcome to take care of them in our office when you arrive.
